Best-in-Class Background Screening
I authorize Trailways Dispatching Transportation to obtain my driving and background records for screening per Section 391.23 of the FMCSR. I release all parties from liability related to this request.
As per the Fair Credit Reporting Act and related laws:
- I authorize the acquisition of this report in writing.
- I have received a separate disclosure about obtaining a consumer report for driver qualification.
- The information will only be used for lawful CMV driver qualification purposes.
- It will not be used in violation of any equal opportunity laws.
- Before any adverse decision, I will receive a copy of the report and my consumer rights.
